Below Knee Intervention with Balloon Angioplasty

Relevant clinical history and physical examination
A 80 year old woman was admitted with increasing size of abdominal aortic aneurysm (44.2 ¡æ 49.5 mm for 1year). She had been treated with coronary artery disease and hypertension.
Relevant test results prior to catheterization
A CT scan showed 49.5 mm sized, fusiform shaped infrarenal aortic aneurysm.
Relevant catheterization findings
Right anterior and posterior tibial arteries were occluded totally. Right peroneal artery was diffusely narrowed up to 60-80%. (Figure 1, Figure 2, Figure 3, Figure 4, Figure 5)
Procedure
A 6F sheath was inserted anterograde through right femoral artery. The 0.014 inch Fielder XT guidewire (180cm length) was inserted into Rt. peroneal artery lesion and then balloon angioplasty was performed with POWERFLEX 2.0 x 150 mm balloon. (Figure 6) As next step, we performed balloon angioplasty with POWERFLEX 2.5 x 60 mm balloon in Rt. peroneal artery for 2 minutes. (Figure 7) The final angiogram showed good blood flow. ( Movie 1)
