Case Presentation |
The patient was a 68 year old man with claudication of both legs. He received a PCI at LM to middle LAD about 18 months ago. His cardiovascular risk factors were diabetes, hypertension, hyperlipidemia, chronic kidney disease. |
Relevant Test Results Prior to Catheterization |
Segmental limb pressure with Doppler revealed significant decreases of ankle-brachial indicies and pressure drop at the level of both ilio-femoral segments and femoro-popliteal segments. |
Relevant Angiography Findings |
The peripheral angiogram showed severe stenosis of both common iliac arteries(CIA) with dissection flap ( Movie 1, Movie 2). |
Procedure |
Two 7F sheaths were inserted into both femoral arteries, respectively. We passed through the stenotic sites of both CIA using a 0.032 soft terumo wire with 5F JR4 diagnostic catheter and replaced with a 0.035 Amplatz stiff wire for stable back up during intervention on both sides( Movie 3). 9 x 25mm Expresss LD stent was implanted at Rt CIA first and then 9 x 37mm Express LD stent was implanted at Lt CIA. Both stents was inflated up to 9mm (8atm). Post-stenting dilation was not done. Final angiogram showed well-positioned stent in both CIAs ( Movie 4). |
