Carotid Artery Stenting in left internal carotid artery stenosis

A 78-year-old man was admitted with the severe stenosis of left carotid artery. A MR angiogram showed a significant stenosis of Lt ICA with eccentric isoechoic plaque. She had a history of hypertension, and hyperlipidemia.

Baseline coronary angiography

A Carotid angiogram showed significant stenosis in the Lt ICA. (Figure 1)

Procedure

A 8 Fr sheath was inserted into right femoral artery. Left carotid artery was engaged with Rt. JR4 guiding catheter carotid and cerebral angiograms were taken. ( Movie 1) A distal embolic protection device (Filter Wire EZ) was placed in the distal portion of Lt. ICA. A 10*24mm Carotid Wallstent was deployed. (Figure 2) Adjunctive postdilatation was performed with a 6.0*20mm Ultra Soft balloon at 6atm. All procedure was uneventful. Final carotid angiogram showed good result with no residual diameter stenosis. ( Movie 2)
